Choosing the Right Provider!

Finding a provider that fits your needs is important. With your dental insurance, you have the freedom to select the provider of your choice. Explore our online provider directory to find an in-network provider in your area. Simply follow these steps:

1. Locate the Provider Quick Search (upper right corner of your screen), select your Network from the drop-down menu.

2. Enter your Zip Code or Address, City and State, then click Search.

Don't know your Network?

Before you search for a provider, be sure to check the back of your dental ID card to locate your network name.
